DOH Changes the Procedures for Certified Food Manader ID Cards

DOH is no longer issuing the certified food protection manager ID with photo. Certificates are now being issued that have to be displayed in the food establishment. For those with current certified food protection manager ID’s it will be recognized until their expiration dates. NOTICE To The PUBLIC
The Department of Health (DOH) Food Safety and Hygine Inspection Services Division has changed the procedures of the Certified Food Manager Identification Cards.

Effective immediately, DOH will no longer issue identification cards with photographs.

The Department of Health will issue Certificates to members of the public in recognition of successfully passing a national food manager certification examination.

To obtain a DOH-issued Certified Food Manager Certificate, provide:
• Original certificate from the training entity that indicates successful completion of course and examination with the test scores
• Two (2) forms of government issued ID (one with a picture)
• Check or money order for $35.00, payable to DC Treasurer. NO CASH

When all required documents are submitted, DOH will issue a certificate with two parts. Hang large section of certificate in a conspicuous location. Cut wallet size certificate, laminate. Have available during business hours.

DOH-issued Certificate &  photo ID must be available for inspection.
